A copy of your report, for your records

To protect your privacy, we verify every request against our records and a staff member reviews it before any report is released. Reports are provided only to the applicant, or to a parent/guardian for children.

Every part of your name — all given names and all family names — in English letters, exactly as on your passport or ID. No accents.

Use only the number printed in the top-right corner of the sheet.
IME: 8 digits (e.g. 12345678) — do not type “IME”.
UMI: the letter U then 9 digits (e.g. U012345678) — that is a zero after the U, not the letter O.

No problem — you can still order. Our staff will find your case from your name and date of birth, and an order without a case number is always reviewed by a staff member before anything is sent. This can add a little processing time.

You will be taken to Stripe to pay — by card, Apple Pay or Google Pay. Payment is taken first; your record is released only after we verify your identity and a staff member approves it, and is refunded in full if it is not.

Stripe will ask for an email address for your payment receipt. That is only for the receipt. Your record is still sent to the email address on your office registration, and nowhere else.
