How this website handles your information
This page describes what our website collects and why. It is about the website only — information you share during a clinic visit is handled under Ontario health-privacy law (PHIPA) and our in-office consent process.
What this website collects
- Ordering a record copy: your full name, date of birth and IME/UMI case number — used only to verify your identity against your exam record before anything is released. Payment is processed by Stripe on Stripe's own pages; card details never touch our website. We do not ask for a delivery email: your record is sent to the address on your office registration.
- Checking your status: the IME/UMI number and date of birth you enter, used for that lookup.
- Booking: appointments are made through YouCanBook.me, which collects the details you enter in its booking form under its own privacy policy.
- Contacting us: the name, contact details and message you send through the contact form, delivered to our clinic inbox so we can reply. Please don't include medical details there.
- Bot protection: our forms use Cloudflare Turnstile, which sets a cookie to distinguish people from automated abuse.
Analytics and cookies
We do not load analytics unless you choose Accept on the cookie notice. If you accept, we use Google Analytics to understand how the site is used; if you decline, or make no choice, no analytics load. Your choice is stored on your device and you can change it any time via . Pages where you enter personal details for orders or status checks never carry analytics, regardless of your choice.
Where information is processed
Payments are processed by Stripe, order records are maintained using Google services, and contact-form messages are delivered through Web3Forms; these providers may store data in the United States. Report files remain in the clinic's internal systems.
Retention
Order and disclosure records are kept for six years, consistent with our professional and tax obligations. Identity details submitted for verification are not kept on the website beyond what that verification requires.
